Clinical Efficacy of Ki-67, Apoptotic Index, bcl-2, and p53 in Bladder Tumor: Correlation with Stage, Histologic Grade and Recurrence Rate

Abstract
Purpose: Clinical parameters such as stage and histologic grade are generally used indicators of prognosis in bladder cancer, but not accurate enough to select treatment modality or predict recurrence and progression. To assess the clinical significance of biological tumor markers(Ki-67, Apoptotic index, bcl-2 and p53), we have studied the correlation of these biological tumor markers with clinical parameters and the recurrence rate of superficial bladder tumors.

Materials and Methods: We prospectively studied 72 patients with transitional cell carcinoma of bladder who underwent operation from April 2001 to December 2002. Associations of Ki-67, Apoptotic index, bcl-2 and p53 immunoactivity with stage, histologic grade and recurrence rate of superficial tumors were investigated.

Results: Tumor stage were superficial in 45 patients and invasive tumor in 27 patients. Thirteen patients proved to be grade 1, 24 grade 2 and 35 grade 3. This study revealed a significant correlation between Ki-67 positivity and histologic grade and stage(p£¼0.01). There was statistically significant correlation between AI and p53 positivity and histologic grade(p£¼0.05). The mean follow-up was 12 months. We could not find statistically significant correlation between these biological tumor markers and recurrence rate.

Conclusions: As a part of studies about clinical efficacy of biological tumor markers, our study revealed significant correlation between biologic tumor markers(Ki-67, Apoptotic index, p53) and clinical parameters. The correlation between these biological tumor markers and disease course could be revealed with a larger number of patients and longer follow-up.
